Family Retreat Registration Form

Use this form to register for the St. Luke Family Retreat, which will take place September 11-13, 2009, at Camp Nawakwa. The registration deadline is June 30.

Permission Slip

This form is required in order for a child or youth to be able to participate in a KFC or SLY event away from the church.

DPW Background Check Request Form

Each volunteer who will work with our children must complete this form and give it to Lynn Fry, so it can be mailed in along with the church's check. The background check can take up to two months. The report will be mailed to the individual volunteer's home, and it should be brought to church so Lynn can give the volunteer a copy and file the original.
